New Patients

New Patient Registration 

Lower Clapton Group Practice is part of Safe Surgeries Network which means, as a practice, we are committed to providing equal access to our services and welcome new patients who live within our practice area, regardless of their immigration status. 

What does this mean to a patient? 

When registering with our practice, patient will not be asked to provide a proof of address nor a proof or their identity. We will only confirm that the address provided is in our practice catchment area.

As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are asked to undertake a new patient health check.

Getting this completed and entering your details onto the clinical software may take up to 30 minutes, and we ask that you attend between 12:00-14:00 or 18:00-19:00.

Medical treatment is available from the date of registration.  Please contact reception for further information.

New Patient Health Check

It is a requirement of our practice that all newly registered patients aged 6 and over undertake a new patient health check. Parents of children under 6 will be required to supply us with the details of their children’s immunisation status (available in your red baby book).

The new patient health check will assess some aspects of your health and lifestyle and check some aspects of your physical health. The check takes place either in an appointment with a health care assistant or via our electronic SurgeryPod (which does not need an appointment).
