Neighbourhoods health and wellbeing programme launches new website!
City & Hackney are pleased to announce that they have a new Neighbourhoods website and logo!
Neighbourhoods are continuing to bring local residents, community groups and services together within our local communities to increase preventative partnership working at a local level.
There’s increasing evidence to suggest that working collaboratively across sectors and in partnership with communities like Neighbourhoods, is key to improving health and reducing health inequality.* Additionally, the Fuller review on Primary Care Networks (networks of GP practices) and their future within Integrated Care Systems has just been published last week and recommends that all PCNs should evolve into ‘integrated neighbourhood teams’ in a bid to improve services.
You can now easily find out more about how the City & Hackney Neighbourhoods are working with local communities to make the most of their unique assets and address health inequalities through information sharing and collaboration at the new Neighbourhoods website.
