At Lower Clapton General Practice, we are committed to providing safe, respectful, and inclusive healthcare for all our patients, including those who are trans, non-binary, or gender-diverse. We recognise the inequalities and barriers that trans people often face in accessing timely, affirming healthcare, and we strive to ensure that everyone who comes to us is treated with dignity, compassion, and respect.
Our Current Approach
In light of recent updates to guidance from the General Medical Council (GMC) and statements from the Royal College of General Practitioners (RCGP), we have reviewed and updated our approach to the prescribing and monitoring of gender-affirming treatments.
Patients Already Under Our Care
For patients whom we have already initiated on gender-affirming treatment under shared decision-making, we will continue to provide ongoing prescriptions and care in line with previously agreed treatment plans. We will ensure continuity, safety, and support in managing these treatments, and continue to offer monitoring and follow-up as appropriate.
New Requests for Gender-Affirming Treatment
In accordance with current GMC guidance, we regret that we are no longer able to initiate new prescriptions for gender-affirming hormone therapy unless recommended and supervised by a specialist service, such as an NHS Gender Identity Clinic (GIC) or an appropriately qualified private provider.
We will, however, continue to:
- Provide referrals to NHS GICs and support patients during the often lengthy waiting periods.
- Offer monitoring and general health care for patients receiving treatment from specialist services.
- Respect and affirm each patient’s gender identity in all aspects of their care.
- Provide mental health support and signposting where helpful.
- Advocate for patient wellbeing and equitable access to care, within the boundaries of regulatory guidance.

While these changes affect how we are permitted to prescribe, they do not change our commitment to trans and non-binary patients. We remain a practice rooted in inclusion, compassion, and patient-centred care.
