Information about your GP practice – you do not need to take any action

We are writing to you as a patient, carer or guardian registered at Lower Clapton Group Practice, to inform you that the practice will be relocating to a new purpose-built healthcare facility on Monday 13th May 2024.

Lower Clapton Group Practice located at 36 Lower Clapton Road, London, E5 0PQ will be moving to The Portico, 34 Linscott Road, London, E5 0RD. Further to consultation with our patients, the practice will be known as the ‘Lower Clapton General Practice’ following the move.   

All of our practice team who work at our current site will transfer to the Lower Clapton General Practice, The Portico, thus ensuring continuity of care within the new facility. The new building is scheduled to open to patients on Monday 13th May 2024.

Our current site at 36 Lower Clapton Road will remain open as usual until the end of Friday 10th May 2024. Please continue to contact us on the normal telephone number and we will advise you on which site you need to visit if a face-to-face appointment is required. The practice telephone number will remain the same on: 020 8986 7111 and the practice email address will also remain unchanged: [email protected]

Background

The move to the landmark site at The Portico, 34 Linscott Road, will allow this long-established GP Practice, one of the largest in Hackney, to function from a purpose built facility, providing services for our growing patient population, additional space for our workforce and modern facilities to cater to the needs of our patients.

It will:

  • Provide 25 consultation rooms, all situated on the ground or lower ground floor, allowing easy access to those with mobility issues.
  • Accommodate a larger patient list making it more sustainable for future population growth.
  • Collaborate with other practices within the Primary Care Network to share workforce, resources and expertise, for an overall better patient experience

What does this mean for you as a patient?

You will continue to be registered as a patient with the practice. There is no action for you to take. The move will not affect the care you receive and you will be able to continue to access all of the services we offer.

We will still be responsible for providing your care and will be able to access your medical records from the new location and during the relocation period.

The practice will be arranging a patient engagement event following the move. The event will provide you with the opportunity to ask any questions and explore the new facilities. Further details about the patient engagement event will be made available online: www.lowerclapton.nhs.uk/ closer to the time.

We will make every effort to ensure minimal disruption to services during the relocation period and will notify you if we anticipate any delay or issues as soon as possible.

Location of the new premises

A map of the new location has been enclosed, which includes travel directions from the existing site to the new premises. The new site also has:

  • One disabled parking bay
  • Buggy spaces within the practice
  • Bike rails outside the premises.

Please note that the new premises will not have any patient parking facilities.
